Why do patients that have kidney disease or are on dialysis have a low blood count?

When your kidneys are diseased or damaged, they may not be making enough of a hormone called erythropoietin. Erythropoietin tells your body to make red blood cells. A lack of or decreased amount of this hormone may lead to anemia.
