For a copy of your personal health information held by Medicare, send the completed, signed authorization to: Medicare BCC, Written Authorization Dept P.O. Box 1270 Lawrence, KS 66044 Your letter must include:

  • Your name,
  • Your Medicare Health Insurance Claim Number (the number on your Medicare card),
  • Your date of birth,
  • Your return address,
  • Your telephone number in case more information is needed, and
  • Your signature.

Note: Medicare doesn't have copies of your medical records. You have the right to ask for a copy of your medical records from your health care provider. You may also ask that a copy of your records be sent to another provider, an attorney, or another person of your choice.
