You can name a relative, friend, advocate, doctor, or someone else to act for you. Others may be authorized under State or other applicable law to act for you. If you want to name someone to act for you in an appeal, an Appointment of Representative (AOR) form (Form CMS-1696) can be found at http://www.medicare.gov/Basics/forms or by calling 1-800-MEDICARE.
