The information for my nursing home is incorrect on your Website. How can I get it corrected?

The data contained in this Website comes from 2 data sources: The Centers for Medicare & Medicaid Services' (CMS) Online Survey, Certification and Reporting (OSCAR) database and the National database known as the Minimum Data Set (MDS) Repository. The data extracted from OSCAR includes the nursing home characteristics, inspection results, and five resident characteristics (including Residents Who are Very Dependent in Eating, Residents Who are Bedfast, Residents With Restricted Joint Motion, Residents With Unplanned Weight Gain or Loss, and Residents With Behavioral Symptoms). This database is updated by the State Survey Agencies. The data extracted from MDS includes three resident characteristics (Residents With Physical Restraints, Residents with Pressure (Bed) Sores, and Residents With Bowel or Bladder Incontinence). This data is submitted by each nursing home and is entered in the MDS repository. If your information is incorrect, or out of date, please contact your State Survey Agency to have your information corrected. Their phone number is listed in the Nursing Home Compare search results pages, or you can find their phone number in the Helpful Contacts section on Medicare's Website. We update Medicare's Website every month. Your information will be corrected online the month after OSCAR and MDS is updated.
